Money Follows the Person Program
What is Money Follows the Person program?
Assist older adults and people with disabilities move out of nursing homes and back into their communities.
In the 10 county area that we serve Alternatives for the Older Adult Care Coordinators will work with you to determine if you might benefit from this program. The process begins with a visit to you in the nursing home.
What criteria must you meet to eligible for this program?
- resided in a nursing home in Illinois for a minimum of 90 days
- have received Medicaid benefits for a minimum of one day
- are 60 years of age or older
What are the goals of the program?
- increase use of home and community based services long term care services
- eliminate barriers that prevent or restrict flexible use of Medicaid funds for long term care services in the settings reflecting individual choice
- increase the ability to assure continued community based long term care services to eligible people after transition
- ensure quality assurance and improvement continuously occurs
