Medication Management
Medication Management is available in our 10 county area of Bureau, Henderson, Henry, Knox, LaSalle, McDonough, Mercer, Putnam, Rock Island and Warren County.
Medication Management services can assist older adults to remain in their home. Care Coordinators from Alternatives for the Older Adult (AFOA) are trained to educate and assist older persons to use medications properly, to manage health problems, and to prevent incorrect medication use and adverse reactions.
Clients throughout the ten county area who have a need for Medication Management will be identified by AFOA Care Coordinators during their comprehensive evaluation for services, either at home, in the hospital, or when assisting in the deinstitutionalization process.
