Assessment of Need for In-Home-Services

Alternatives for the Older Adult is the Case Coordination Unit for the 10 county area of Bureau, Henderson, Henry, Knox, LaSalle, McDonough, Mercer, Putnam, Rock Island and Warren County. They provide the following programs:

Comprehensive Care Coordination: 
A case manger will assess a person’s situation and circumstances and their ability to live independently in the community. It includes a review of the person’s environment in the community, physical, cognitive, psychological and social well being. Identification of existing support systems and the need for further evaluation by other disciplines will be made by the case manager. They will then identify services that will assist the person to maintain maximum possible independent living. Services will be offered and the person may accept or reject the suggested services. In conjunction with the person a plan of care will be agreed upon. Services will be arranged and referrals made to the necessary support systems based on the agreed plan of care.
