Alternatives For the Older Adult
Alternatives for the Older Adult is the Care Coordination Unit for Bureau, Henderson, Henry, Knox, LaSalle, McDonough, Mercer, Putnam, Rock Island and Warren counties. They offer the following services:
Care Coordination Program: offers a comprehensive assessment of the individual and then assists the individual to develop a plan of care to meet their needs.
Community Care Program: offers in home services (homemaker, adult day services and/or emergency home response system) to older adults age 60 and over who meet eligiblity criteria.
Elder Abuse and Protective Services Program: receives and investigates abuse of people 60 years of age and assists with developing a plan to alleviate the abuse.
Money Follows the Person Demonstration Program: assists eligible older adults age 60 and over to move from long term care facilities back into their local community.
Money Management Program: matches trained and bonded volunteers with seniors who need help managing their finances. The volunteers can organize and pay bills, establish and maintain budgets, assist in applying for financial programs and assist with setting and achieving financial goals.
Long Term Care Ombudsman Program: advocates for older adults age 60 and over who reside in licensed long term care facilities (nursing homes, assisted living, supportive living, shelter care and group homes).
