                 Aging and Disability Resource Network

What is the Aging and Disability Resource Network?

The network brings together many human service organizations to meet the need of the increased demand for long term services and supports in local communities for seniors and people with disabilities. There is a great need for streamlined access and service delivery for people needing assistance.

The network will provide easy access to coordinated points of entry from any agency within the network. This will provide more efficient coordination between human service agencies. The person seeking service from any agency within the network will have access to quality timely referrals.

Information & Assistance to seniors, family caregivers, people with disabilities regarding available services

Referral to needed services.

Individual options counseling, that explain what long term care options are available within the local area with a full explanation of what each service will deliver.

Benefits Checkup, a data base that will provide an overview of what federal and state programs each individual may be eligible will be utilized.  Assistance and referral will then be made to the programs an individual chooses to apply for. 

Specialized Care Coordination,
may be provided as needed from various agencies participating in the network.

There will be regional networks created throughout the 10 county service area.
